HPAE members have the option to participate in, or opt out of, the CVS Maintenance Choice program. This applies to 90 Day prescription refills.

PLEASE NOTE: ALL 90-DAY SUPPLY “MAINTENANCE DRUGS” CAN ONLY BE FILLED THROUGH THE CVS MAINTENANCE CHOICE PROGRAM, AT CVS PHARMACY, OR CVS CAREMARK MAIL SERVICE. TO OPT-OUT OF THIS PROGRAM, 90-DAY DRUG SUPPLIES WILL ONLY BE DISPENSED IN 30-DAY QUANTITIES, WITH THE APPLICABLE 30-DAY CO-PAY FOR EACH REFILL. THE OPT-OUT PROVISION APPLIES TO HPAE MEMBERS ONLY.
