HPAE FXB/CHU Healthcare Case Manager Survey Now On-line

Please take a few minutes of your time to fill out the survey described here. It is intended for all Health Care Case Managers in the FXB Child Health Unit represented by HPAE Local 5094. This survey is not intended for Staff Assistants. The purpose of the survey is to get a better understanding of the impact of recently added job duties. It should take around 10 minutes to complete.

The results of the survey will be analyzed by your Local 5094 Impact Bargaining Team in order to support ongoing negotiations with the University over these duties. No identifying information will be shared with Management. Only statistical information in the aggregate will be presented in the context of negotiations. We ask for your name only for the purposes of eliminating duplicate surveys and preventing potential falsification.

This survey should not be completed by supervisors or managers.
