Important: Worksheet for Potential Monies Owed to Members

Attached please find a worksheet to help identify any
wages, benefits, or other monies that may be owed to you as a result of the
unilateral changes implemented by management and the ongoing Llanfair
House/Windsor bankruptcy proceedings.

It is important that you complete and return this worksheet if you believe you may have experienced any financial loss, including unpaid
wages, lost benefits, health insurance issues, vacation or PTO losses, or other
work-related expenses or losses. The information collected will assist the
Union in identifying and preserving potential claims on behalf of our members.

Because there are important deadlines associated with the
bankruptcy process, we ask that you complete and return the attached worksheet no
later than June 22, 2026, so that the Union can timely review and submit
any potential claims.
