Workplace Violence Reporting Form

The workplace violence reporting form is used to report incidents of violence in the workplace. Workplace violence is a major cause of work-related injuries for nurses and other healthcare workers. Each facility in New Jersey is required by law to have a policy in place to prevent workplace violence and must establish a joint committee on workplace violence prevention.
